Gonstead Chiropractic Malaysia: Assess First, Adjust Second
Most adjustments are general — a region is moved and the practitioner works from a broad impression. The Gonstead method inverts that. Five separate findings have to agree on which single segment is at fault before anything is adjusted, and if they do not agree, the answer is more assessment rather than a bigger adjustment. What the Gonstead Method Actually Changes
Developed by Clarence Gonstead over roughly five decades of clinical practice, the Gonstead system of chiropractic is not a different way of moving a joint. It is a different standard of proof about which joint to move — and, just as often, which joints to leave alone.
The problem it was built to solve. Pain rarely announces its own source. A stiff, aching neck is frequently driven by a segment several levels below it, and the spot that hurts is the spot compensating rather than the spot failing. Adjust where it hurts and you get an hour of relief followed by the same complaint next week. The Gonstead method exists because that pattern is common enough to be predictable.
The method rests on a specific idea about the intervertebral disc. Gonstead held that the disc, not the vertebra alone, is the structure that determines whether a spinal segment moves properly, and that a segment losing its normal relationship with its neighbours produces effects that can be located precisely rather than guessed at. Whether or not every part of that model has held up in the intervening decades, its practical consequence is unambiguous and useful: the practitioner has to justify the adjustment before delivering it.
That justification comes from five findings taken independently and then compared. If they converge on the same level, that level is adjusted. If they contradict each other, the contradiction is the finding — and it usually means something is going on that an adjustment is not the answer to.
The five findings
- Visualisation — posture, gait and how you hold yourself while you describe the problem
- Instrumentation — a heat-differential scan run down both sides of the spine, looking for asymmetry
- Static palpation — the spine felt at rest, for tone, swelling and temperature change
- Motion palpation — each segment moved individually to find where movement is genuinely restricted
- X-ray analysis — of scans you already have, read for structure rather than for pathology screening
What that means on the day
A first Gonstead visit is slower than you expect, and most of it is not treatment. Expect roughly forty minutes before anything is adjusted: history, the scan, palpation with you seated and again lying down, and a review of any imaging you brought. The chiropractic adjustment itself takes seconds.
People sometimes read the ratio as poor value. It is the opposite — the adjustment is cheap to deliver and easy to get wrong, and the forty minutes is what stops it being a guess. If you have had adjustments before that felt good for a day and then wore off, this is the part that was missing.
How the Adjustment Is Delivered
Three things distinguish a Gonstead adjustment from a general manipulation: where the contact is made, how the patient is positioned, and how little of the spine is involved.

Had Adjustments Before That Did Not Hold?
That is the most useful thing you can tell us. Where relief lasts a day and the same complaint returns, the segment being adjusted is usually not the segment at fault — and that is a question the assessment answers.

What people ask about the Gonstead method before their first assessment.
Mainly in what happens before the adjustment. A general manipulation treats a region; Gonstead requires five separate findings — posture, a heat-differential scan, static palpation, motion palpation and X-ray analysis — to agree on one specific segment first. The adjustment is then delivered to that segment alone, usually with you seated or side-lying rather than rotated flat.
The adjustment itself is quick and most people describe pressure rather than pain. You may hear a pop — that is gas releasing from the joint, not bone moving on bone, and its absence does not mean the adjustment failed. Mild soreness for a day or two afterwards is common, in the way it is after unfamiliar exercise.
Not to be seen. Trinity does not take X-rays on site. If you already have scans from a hospital, GP or previous clinic, bring them — they are read as part of the assessment and they change what is appropriate to adjust. Without them the examination proceeds on the other four findings, and you will be referred for imaging if what is found warrants it.
Budget around an hour. Most of that is assessment: history, the instrument scan, palpation seated and lying down, imaging review, and an explanation of what was found before anything is done. Follow-up visits are much shorter because the assessment narrows to a progress check on the segment being treated.

That is decided after the assessment, not before it, and nobody should quote you a number over WhatsApp. What is reasonable to expect is a plan with a stated review point — a set of visits, then an honest reassessment of whether it is working. If nothing has changed by the review, the plan should change rather than simply continue.
No, and the assessment exists partly to establish that. Active inflammatory arthritis, certain fractures, bone disease, some vascular conditions and a number of other presentations make adjustment inappropriate. If your findings fall into one of those groups you should be referred rather than treated, and pregnancy is handled through a different protocol entirely.
